Informed Consent to Telehealth

Telehealth is the delivery of health care using secure video, audio, and messaging instead of an in-person visit. Your clinician is licensed in the state where you are located at the time of the visit.

Your visit is documented in your medical record exactly as an in-person visit would be.

Benefits include faster access to a licensed clinician, continuity with the same care team, and care from home.

Limitations include the inability to perform a hands-on physical exam. Some conditions require in-person evaluation, imaging, or procedures. Your clinician may determine that telehealth is not appropriate and refer you elsewhere; this is a clinical judgment, not a denial of service.

Risks include technical failure or poor connection quality delaying or interrupting care, information transmitted electronically being insufficient for a clinical decision, and — despite our safeguards — the rare possibility of a security breach affecting your information.

Delays in evaluation or treatment can occur because of equipment or connectivity problems.

We do not record video visits. Clinical documentation of the visit is written by your clinician and stored in your record.

Only people necessary to your care participate in the visit. You may ask anyone else to leave, and you may request a chaperone.

Labs and diagnostics, when ordered, are performed by an independent laboratory. Results are returned to your clinician and released to you with interpretation.

Your participation is voluntary. You may withhold or withdraw consent at any time without affecting your right to future care, and you may request an in-person referral instead.

You have the right to know who is participating in your visit, to ask questions before agreeing to any treatment, and to receive a copy of your record.
